INFORMED CONSENT
Please read this and indicate if you give your informed consent.
By reading this statement and accepting the terms, I willingly volunteer to participate in
this research study, conducted by Susan Wilde, a doctoral student of
clinical psychology at John F. Kennedy University, Orinda, California. I
understand that this study will look at interventions by witnesses in child
maltreatment situations as they are happening. My participation will
include filling out a brief demographic questionnaire, and then a more
in depth set of answers to questions or an online interview that will take approximately 30 minutes to one
hour to complete.
I understand that my participation in this study is completely voluntary. I
may stop participating at any time without problems.
My participation in this study and my responses in the interview will remain
confidential and will not be identified or linked to me in any written
report or oral presentation of results of the study. My identity will be
coded to a unique number and I have the option of remaining anonymous
throughout the research process. The list of links between actual participant identities and codes
will be kept separate from all other research data, such as the audio tapes
of live interviews and their written transcripts, and data analysis records.
However, I understand that the researcher has a legal obligation to report
certain acts to the authorities, including child abuse, elder abuse, or a
credible threat by me to my own safety or the safety of others.
I understand that I have no financial responsibility in this study, and that
I will not receive financial renumeration. I understand that the interview
will explore a conflictual situation that I have already experienced, and
that talking about it may bring up upsetting feelings. I also understand
that, if after my interview I experience distress as a result of my
participation, I may request from the researcher a list of public and
private support resources in my community. The purpose of this study is to
learn about what works in child abuse interventions and to design a class to
teach people how to perform effective interventions in child abuse
situations.
If I have any questions regarding this study, I may contact the researcher
Susan Wilde at (510) 704-7715 or I may call the thesis advisor Dr. Jimmie
Turner at (925) 254-0100. If I have any questions regarding my rights as a
participant in this study, I may contact the PsyD Research Committee, John
F. Kennedy University, Orinda, California, at (925) 254-0110.
